In order to activate the SCS Limited Warranty, this form must be submitted as well as the SCS Warranty Registration Form.

In conjunction with, and to activate the SCS Enclosed Shaft System Limited Warranty, I hereby certify the vessel engine hours on which the SCS system is installed, to be correct as of the date specified below.
